Aging brings change — sometimes slowly, sometimes all at once. Sound Transitions Care at Home, LLC exists to help older adults and families make thoughtful decisions, reduce stress, and find the support that truly fits.

 

We’re a local homecare company led by Marianne Ward, a Registered Nurse and Certified Geriatric Care Manager, and we bring both clinical perspective and real-world practicality to every conversation.

About Marianne Ward, RN, CMC 

Marianne is a licensed Registered Nurse and Certified Geriatric Care Manager (CMC) through The National Association of Certified Care Managers and has over 20 years of clinical experience. She has helped families navigate the complex world of geriatric care, make informed, educated choices, and maximize their quality of life.

 

Marianne's expertise includes eldercare, post-acute care planning, conservatorship, and mental health advocacy. Having cared for her own parents, Marianne understands the diverse evolving needs inherent in aging including ensuring the safety, physical, emotional and spiritual complexities of caring for an elderly loved one whether at home or in other settings.

Our mission is to provide family members, caregivers, and aging loved ones life-improving solutions and guidance for the challenges that can come with aging in place — including health, social, legal, and personal needs.

We use an informed, compassionate, hopeful and personalized approach. That means we listen closely, assess what's happening and help you move toward a plan that supports:

What is a Geriatric Care Manager?

A Geriatric Care Manager assists clients in attaining thier maximum quality of life and potential by coordinating care and support services, providing information, advice and resources to the families of older adults. Geriatric Care Managers address a broad range of transitional issues related to the well-being of their client and share their extensive knowledge about the costs, quality and availability of resources in their communities.
